Envelope of Life

West Deer EMS is excited to offer this life saving form, now available online. During an emergency, there are times when patients are unable to speak or think clearly, impeding the emergency personnel’s efforts to help them. The lack of information may waste valuable time. The Envelope of Life is an easy-to-complete form that provides life-saving information to emergency responders.

The completed form is placed in a location easily identifiable to EMS personnel, most commonly on the refrigerator door.

Residents are encouraged to either fill out and print or print and fill out the form (the form is available to be typed into on the computer). Once completed place it on your refrigerator and update every 6 months or when changes need made. Our staff is available to assist anyone needing help in filling out the forms. Remember that this can save valuable time when our crews are assisting your loved ones.

This program is the start of our “Staying Well Together” initiative which will assist our medically vulnerable residents in the prevention of illness or injury.
